Fatty liver disease has quietly become one of India’s most common health concerns, and yet, most people don’t realise how reversible it can be when addressed early. You don’t need extreme cleanses or pricey ‘detox’ plans. What you truly need is clarity: what to eat, what to avoid, and how to support the liver as it heals itself.
To understand the science behind it, we spoke to Dr Vinay Dhir, Chairman, Institute of Digestive and Liver Care (IDL Care) at S L Raheja Hospital, Mahim, Mumbai (a Fortis Associate). He emphasises one simple truth, “Diet is your first-line treatment. Most patients improve far more with consistent food choices than with any medication.”
Dr Dhir explains that non-alcoholic fatty liver disease (NAFLD) is virtually silent in its early stages, and most people discover it only during routine scans. But this is also when the liver is most responsive to change.

A modest 5–7% weight loss can significantly reduce liver fat, calm inflammation, and improve enzyme levels. “We see dramatic improvements within 3–6 months when patients stick to a structured diet,” he notes.
A Mediterranean-inspired pattern remains the gold standard. Dr Dhir suggests:
Half the plate: vegetables, raw or lightly cooked
One quarter: whole grains like brown rice, dalia, millets
One quarter: protein, dal, chana, curd, paneer, eggs, or fish/chicken
Use oils such as mustard, groundnut, or olive oil sparingly. Portion control is crucial, skip second helpings and avoid sugary desserts.
Whole fruits are strongly protective because they offer fibre, antioxidants and slow sugar release. Dr Dhir recommends 2–3 servings daily, preferably spaced through the day. Ideal choices include:
He advises strictly avoiding fruit juices, including fresh ones, “Juices deliver concentrated fructose directly to the liver and worsen insulin resistance.”

These reduce inflammation, stabilise blood sugar, and support liver regeneration.

Crash diets and trendy detox programmes. “Your liver doesn’t need cleansing; it needs relief from sugar, refined carbs and fried foods,” Dr Dhir says. Rapid weight loss can worsen inflammation, so aim for 0.5–1 kg per week instead.
Pair your diet with 30 minutes of brisk walking or moderate exercise most days of the week. This boosts insulin sensitivity and speeds up the reduction of liver fat.
Fatty liver disease is highly reversible, but only if you act early. With consistent eating, mindful portions, regular walking, and a commitment to whole foods, the liver has an extraordinary ability to repair itself.
As Dr Vinay Dhir puts it, “Early intervention prevents years of complications. The sooner you start, the better the outcomes.”
